Missed a Wegovy, Ozempic, Zepbound or Mounjaro Dose? Label Rules
How late each label lets you take a missed GLP-1 dose: 4 days for Zepbound and Mounjaro, 5 for Ozempic, 2 days before the next one for Wegovy.

Missed your weekly shot? The labels give you a window. Zepbound and Mounjaro say to take it within 4 days (96 hours) of when it was due, Ozempic says within 5 days, and the US Wegovy label says to take it as long as your next dose is more than 2 days away. Miss the window and every one of them says the same thing: skip it and go back to your usual day.
The daily medicines work differently. A missed Wegovy pill, Ozempic tablet or Rybelsus tablet is simply skipped. Foundayo says take it as soon as you can, but never two in one day. Saxenda says don't make it up at all.
Everything below comes from each medicine's own FDA label on DailyMed, quoted or closely paraphrased, with a link to the exact label. It's a summary of what the labels say, not medical advice, and if your prescriber has told you something different, their instructions come first.
The rule for each medicine

The labels are the versions on DailyMed as of October 9, 2026 (Zepbound and Mounjaro SPL version 40, Ozempic 20, Wegovy 19, Rybelsus and Ozempic tablets 14, Saxenda 22, Foundayo 10). Labels do change, so the links go to the live versions.

Why the labels count differently
Read the table twice and you'll notice the weekly labels don't even measure from the same point.
Zepbound, Mounjaro and Ozempic count forwards from the missed dose: so many days after it was due. The US Wegovy label counts backwards from the next dose: as long as that one is more than 2 days off, its "take it" sentence applies.
On a steady weekly schedule, the two semaglutide shots end up in the same place. A week is 7 days, and 7 minus 2 is 5, so "more than 2 days before the next dose" runs out at the same moment as "within 5 days after the missed one". The EU product information for Wegovy simply says 5 days after the missed dose, the same as Ozempic. The two tirzepatide labels are a day shorter at 4 days, and they add a rule the semaglutide labels don't have: never 2 doses within 3 days (72 hours) of each other.
There's one small gap in the Wegovy wording. It covers "more than 2 days away" and "less than 2 days away", but not exactly 2 days. If you're sitting right on that line, ask your pharmacist rather than guessing.
The EU labels
If your pen came from a European pharmacy, the European Medicines Agency's product information applies, and it isn't always the same:
- Wegovy: within 5 days after the missed dose (the US label counts 2 days before the next one).
- Ozempic: within 5 days, the same as the US.
- Mounjaro: within 4 days, the same as the US. The leaflet adds that "the minimum time between two doses must be at least 3 days."
- Saxenda: if you remember within 12 hours of your usual time, take it. More than 12 hours, skip it and take the next one the following day at the usual time. The US Saxenda label has no 12-hour window.

A worked example with real dates
Say your Zepbound dose was due on Monday, October 5 at 8:00 AM, and you realise on Wednesday at 2:00 PM that you never took it.
- How late is it? Monday 8 AM to Wednesday 2 PM is 2 days and 6 hours: 54 hours.
- Which sentence applies? 54 is inside the label's 96 hours, so it's the first one: "If you miss a dose of ZEPBOUND, take the missed dose as soon as possible within 4 days (96 hours) after the missed dose."
- When does that window close? 96 hours after Monday 8 AM is Friday, October 9 at 8:00 AM. From then on, the second sentence applies: skip it and take the next dose on the regular day.
- What about the 3-day rule? Your regular day stays Monday. Wednesday 2 PM to Monday, October 12 at 8 AM is 4 days and 18 hours (114 hours), comfortably more than the 72 hours the label asks for between two doses.
Same dates on the other weekly labels: Mounjaro gives the same answer. For Ozempic, 54 hours is inside 5 days. For Wegovy, the next dose (Monday 8 AM) is 4 days and 18 hours away, which is more than 2 days. All four labels land on the "take it" sentence. Move the clock to Friday noon, 100 hours late, and the tirzepatide labels have flipped to "skip it" while the semaglutide ones still haven't.

Missed more than one dose in a row
This is where you should stop counting and call your prescriber, and three of the labels say so directly:
- Wegovy: "If you miss doses of WEGOVY for 2 or more consecutive weeks, contact your healthcare provider to talk about how to restart your treatment." (Medication Guide)
- Saxenda: "If you miss your dose of SAXENDA for 3 days or more, call your healthcare provider to talk about how to restart your treatment." (Medication Guide)
- Foundayo: "If you miss taking FOUNDAYO for 7 or more days in a row, call your healthcare provider to talk about how to restart your treatment." (Medication Guide)
The prescribing information behind those three tells prescribers to restart at a lower dose and step back up, "to reduce the risk of gastrointestinal adverse reactions." Put simply, after a gap the labels treat going straight back to your old dose as a risk for stomach side effects like nausea and vomiting. Saxenda's prescribing information goes as far as naming the restart dose and schedule. That's written for prescribers, and which dose you restart on is their decision, not something to work out from a label or a blog post.
The Ozempic, Zepbound and Mounjaro labels don't say anything about missing several doses in a row. No rule doesn't mean "carry on as normal". It means the label leaves it to your prescriber, so ask.

Changing your injection day
A missed dose is often the moment people want a new regular day. Take a late Zepbound on Wednesday and Wednesday starts to look like a better day than Monday anyway.
The weekly labels allow a change of day, as long as there's enough time between two doses:

So for a change of day, the US semaglutide labels ask for 2 days and everything else on that list asks for 3. For Zepbound and Mounjaro it's the same 3 days as their "don't take 2 doses within 3 days" rule. If you're moving your day after a missed dose, mention it to your prescriber or pharmacist so your records and refills line up.
The time of day is more relaxed. The Wegovy label says "the time of day and the injection site can be changed without the need for a dosage modification", and Saxenda's says much the same.
Compounded semaglutide or tirzepatide
There's no label rule for these. Compounded products aren't FDA-approved, so they have no FDA label and no missed-dose section to quote. Borrowing the Wegovy or Zepbound rule isn't safe either, because the product, strength and instructions can all be different.
Follow the written instructions from your prescriber and the pharmacy that made it, and ask them if you've missed a dose. The FDA has warned about dosing errors with compounded semaglutide, many of them from mixing up units, millilitres and milligrams. That's a good reason to read the dose off your pharmacy label exactly as written, and never to convert it yourself.
If you took too much
Two doses too close together, or a double dose by mistake: the labels' overdose sections point to the Poison Help line, 1-800-222-1222 in the US. Poison Help is free and confidential, with an online helper too. Their own advice is to call 911 straight away if someone has collapsed, had a seizure, has trouble breathing or can't be woken.
Keep a log so you're not guessing
Most missed-dose panic comes from not being sure. Did I take it Sunday or was that last week? Which side did I use?
