The Medication Guide says

If you miss a dose of ZEPBOUND, take the missed dose as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and take your next dose on the regularly scheduled day. Do not take 2 doses of ZEPBOUND within 3 days (72 hours) of each other.

What the prescribing information tells your prescriber

If a dose is missed, instruct patients to administer ZEPBOUND as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day. In each case, patients can then resume their regular once weekly dosing schedule.

Changing your regular day

The day of weekly administration can be changed, if necessary, as long as the time between the two doses is at least 3 days (72 hours).

Worked example: a Zepbound dose due Monday morning

Say your Zepbound dose was due Monday, October 5 at 8:00 AM and it is now Wednesday at 2:00 PM. That is 54 hours later, inside the 96 hours the label allows, so the page highlights its first sentence: “If you miss a dose of ZEPBOUND, take the missed dose as soon as possible within 4 days (96 hours) after the missed dose.” The label’s window ends Friday, October 9 at 8:00 AM; after that its second sentence applies, and your usual day stays Monday. The Wegovy label counts differently: it looks at how far away the next dose is.

How the page finds the label’s sentence

Each label words its missed-dose rule in its own way, and the page follows the words. The Ozempic, Zepbound and Mounjaro labels count from the missed dose: within 5 days for Ozempic, within 4 days (96 hours) for Zepbound and Mounjaro. The US Wegovy label counts to the next scheduled dose instead: if it is more than 2 days away, take the missed dose; if less, skip it. The daily tablets (the Wegovy pill, Ozempic tablets, Rybelsus) say to skip a missed tablet, Saxenda says to carry on with the next daily dose, and Foundayo says to take it as soon as possible without doubling up.

The page counts hours on your own clock from the date and time you enter, picks the sentence of the label that matches, and shows it highlighted with the rest of the label’s wording around it, plus the prescribing information your prescriber reads. The timeline marks when the dose was due, where the label’s window ends and your usual day. At exactly a label’s boundary (for Wegovy, exactly 2 days before the next dose) the label does not say which side applies, and the page says so.

When more than one dose in a row was missed, a few labels say more: Wegovy (2 or more weeks), Saxenda (3 or more days) and Foundayo (7 or more days) say to talk to your healthcare provider about how to restart. The others say nothing, and the page says that plainly: the label doesn’t say; ask your prescriber. The page never suggests a dose. Compounded products have no FDA label, so it has no rules for them: follow the written instructions from your prescriber and pharmacy.

I missed my Zepbound or Mounjaro shot. How late can I take it?

Both labels say to take the missed dose as soon as possible within 4 days (96 hours) after the missed dose, and if more than 4 days have passed, to skip it and take the next dose on your regularly scheduled day. What does the Wegovy label say about restarting after missing 2 weeks or more?

The Medication Guide says: “If you miss doses of WEGOVY for 2 or more consecutive weeks, contact your healthcare provider to talk about how to restart your treatment.” The prescribing information tells prescribers to restart the dose escalation at a lower dosage, to reduce stomach side effects. Which dose is for your prescriber to decide; this page doesn’t suggest one.

Is the rule the same in the UK and EU?

Not always. The EU product information for Wegovy injection counts 5 days after the missed dose, where the US label counts 2 days before the next one, and it asks for at least 3 days between doses to change the day (the US Medication Guide says 2). Ozempic and Mounjaro have the same missed-dose windows in both, and the EU Saxenda leaflet uses a 12-hour window. Switch to “EU label” above to see the EMA wording.

What about compounded semaglutide or tirzepatide?

Compounded products are not FDA-approved and have no FDA label, so there is no label rule to show. Follow the written instructions from your prescriber and the pharmacy, and ask them if you are unsure. The FDA has warned about dosing errors with compounded semaglutide, often from mixing up units, mL and mg.
